School Health Screenings for 2025-2026
Each year, school nurses are required by the state of Massachusetts to complete various student health screenings (such as vision, hearing, etc) by grade level. If you do not want your child to participate in any or all screenings, you may complete an Opt-Out Form. You only need to complete a form if you do not want your child to have the screening.
**Please note, one form is needed per student.
5th Grade Mandated School Screenings 2025-2026 Opt-Out Form, for vision, postural
6th Grade Mandated School Screenings 2025-2026 Opt-Out Form , for postural
7th Grade Mandated School Screenings 2025-2026 Opt-Out Form , for vision, hearing, growth ht/wt, postural
8th Grade Mandated School Screenings 2025-2026 Opt-Out Form, for postural, SBIRT (or alcohol/substance use)
